Introduction: Since the XIX century changes in design and functions of hospitals began to be implemented, which became singularly important for health care.
Method: An investigation based on the documentation and press of the time was carried out by means of the historical-logical method which allowed the analysis and evolution of Santiago de Cuba hospitals during the first half of the XX century, in their historical context and relationships with the population and health professions.
Development: It was found that at the beginning of the century there was only one hospital in the city, the construction and operation of new centers soon began, which entered into a process of medical hierarchization, with professionalized nursing staff and the incorporation of expensive diagnostic and treatment technologies.
Final considerations: The primacy of hospitals displaced the private healthcare system of the XIX century, centered on doctor´s offices. This initiated a process of hospital-centrism that led to a change in the collective mentality.
