Health Management Model for Pre-Exposure Prophylaxis to the Human Immunodeficiency Virus

Pre-exposure prophylaxis (PrEP) constitutes an effective intervention in the combined prevention of HIV. In Santiago de Cuba, organizational and sociocultural barriers persist that limit its access. A management model was designed and validated to enhance the inclusion and access to PrEP in the municipality of Santiago de Cuba, between July and December 2024. The study included a diagnostic phase and a design phase based on theoretical methods and expert consultation. Validation was performed using Moriyama criteria, applied to 9 specialists selected for their expertise, determined by the Biogram method (K coefficient > 0.8). A systemic model was structured composed of a process management matrix (external) and an areas of incidence matrix (internal), which articulates primary care with multidisciplinary teams. The experts rated the model with the highest score in comprehensibility, justification, and feasibility. The proposed model offers a methodological pathway to decentralize PrEP, focused on the elimination of psychosocial barriers and the training of the basic health team, which ensures the sustainability of the strategy at the primary level.
